INTERVENT International Welcomes New Client: Poplar Care Network
Savannah, Georgia (May 1, 2023) - INTERVENT International is proud to welcome Poplar Care Network ("Poplar") as a valued new US-based provider client. Poplar has partnered with INTERVENT to provide INTERVENT's evidence-based educational content and individual and group digital/telehealth coaching services to support Poplar's lifestyle medicine offerings. This partnership was stimulated, in part, by INTERVENT's peer-reviewed work published in the prestigious Journal of the American College of Lifestyle Medicine.
Founded in 2021, Poplar is a rapidly-growing "high-touch" network of independent primary care lifestyle medicine providers throughout the United States. Poplar delivers cost-effective lifestyle interventions to treat, reverse and prevent chronic diseases, including, but not limited to, cardiovascular disease, type 2 diabetes and obesity. Through team-based care and technology, Poplar and its primary care providers are successfully demonstrating the power of lifestyle medicine to improve the health and wellbeing of their patients. Poplar empowers primary care providers to deliver cost-effective lifestyle medicine programs and be rewarded for their results through shared savings. The Centers for Medicare & Medicaid Services' (CMS) plan to transition Medicare to 100 percent value-based care by 2030 provides a unique opportunity for lifestyle medicine providers to be reimbursed for their results and show the economic value of preventing and reversing chronic diseases.
Lifestyle medicine is a medical specialty that aims to prevent, treat and reverse chronic disease by replacing unhealthy behaviors with healthy ones. Lifestyle medicine is based on six pillars, including:

- Stress management
- Social connection
- Avoidance of risky substances
The prestigious American College of Lifestyle Medicine has added Poplar to its Lifestyle Medicine Corporate Roundtable, which includes more than 50 active member organizations in the lifestyle medicine ecosystem.
